I, the undersigned, hereby authorize Volunteer Background Checker and its designated representatives to conduct a background screening using publicly available records. Upon submission of this application, the following automated checks will be performed using my name, date of birth, and address:

National Sex Offender Registry (NSOPW.gov) — Search of the U.S. Department of Justice national sex offender public database
State Sex Offender Registry — Search of my state's sex offender registry (e.g., Megan's Law in California) via public web records
Family Watchdog — Search of the Family Watchdog sex offender map and registry by name and location
NCMEC Registry — Search of the National Center for Missing & Exploited Children databases
State & County Court Records — Search of state and county court case databases for criminal charges, convictions, protective orders, and domestic violence records
Federal Court Records (PACER) — Search for federal criminal cases via web indexing of U.S. federal court records
CourtListener / RECAP — Search of the free CourtListener federal court records API for cases involving my name
Federal Bureau of Prisons Inmate Locator — Search of the BOP inmate database for current or former federal inmates
Vinelink (State Inmate Search) — Search of state custody and inmate status records across 40+ states
State Dept of Corrections Inmate Search — Search of my state's DOC inmate locator for prison records
SpotCrime — Search of crime incident reports and arrests near my address
CommunityCrimeMap — Search of crime mapping databases for incidents at my location
JailBase (Arrest Records & Mugshots) — Search of arrest records and mugshot databases
Mugshots.com — Search of public mugshot databases
FBI Most Wanted — Search of the FBI Most Wanted fugitive database
US Marshals Most Wanted — Search of the US Marshals Service wanted persons database
Treasury OFAC SDN List — Search of the Treasury Department's sanctioned persons list
Web Search (DuckDuckGo + Bing) — Automated searches across two search engines for my name combined with terms including arrest, charge, conviction, child abuse, endangerment, lawsuit, sex offender, and felony
News Search — Search of news databases for articles mentioning my name in connection with arrests, charges, or criminal activity
Social Media / Public Profiles — Search for my Facebook and LinkedIn profiles to review publicly available content
TruePeopleSearch (Identity Verification) — People search to verify my name, age, address, aliases, and relatives
FastPeopleSearch (Aliases & Address History) — People search to check for aliases, past addresses, and associated persons
Whitepages (People Lookup) — Public records lookup to verify identity, address, and known associates
County Tax Assessor (Address Verification) — Property records search to verify I reside at the address I provided
Voter Registration Lookup — Search of my state's voter registration database to verify identity and address

Each check scans results for 34 red-flag keywords including: arrest, charged, convicted, sex offender, child abuse, endangerment, domestic violence, protective order, restraining order, felony, assault, molestation, pedophile, sexual harassment, stalking, battery, burglary, robbery, weapon, DUI, probation, parole, warrant, fugitive, indictment, guilty, plea, sentenced, incarcerated, inmate, defendant, criminal, theft, and fraud.

I understand that this background check is required for any volunteer position involving children, youth, or vulnerable populations. I understand that the information obtained will be used solely for the purpose of evaluating my suitability as a volunteer and will be kept confidential.

I understand that I have the right to review the results of this background check and to dispute any inaccurate information. I acknowledge that providing false information on this application may result in immediate disqualification.

This authorization remains valid for the duration of my volunteer service and may be renewed annually at the organization's discretion.
